Activation of the vascular endothelium is characteristics of the inflammatory process. Solovey et al demonstrated that in sickle cells anemia, patients have increased circulating endothelial cells that are predominately microvascular in origin (CD36 positive) and most of the EC’s expressed four markers of EC-activation: ICAM-, VCAM-, E-selectin and P-selectin. This data support that vascular endothelium is activated in SCA. Adherence of sickle erythrocytes to microvascular endothelium is posited to initiated vaso-occlusion crisis and the acute chest syndrome. This altered adherence of the sickle erythrocytes to ECs is at least in part related to the EC activation/inflammation characterized by the up-regulation of VCAM-1 and CD36-thrombus protein interactions, and vWf and lipid procoagulants resulting from EC regulated secretion or exocytosis. These processes provide a means by which endothelial cells rapidly and selectively alter the microenvironment of individual vascular beds and modulate the interrelated processes of coagulation, fibrinolysis, inflammation and vaso-occlusion.
