Fingerprint Request Form
NOTICE: The last date to schedule prints for the 2017 year will be December 14th. Print scheduling will resume January 15th, 2018. The University Police Department generally accepts fingerprint requests for university related purposes ONLY. Once you submit your request, someone will contact you by email within 2 business days to schedule a specific date and time for you to come in. All requests must be scheduled in advance. Dates/Times that are requested within 2 business days of submission may not be schedulable.
All fingerprints are taken at the USA Police Headquarters on the main campus at 290 Stadium Blvd off Old Shell Rd just east of Hillcrest Rd. There will be a minimum charge of $15. Only cash or check is accepted. Cash must be in exact change. We do not provide blank finger print cards. Check with your employing department or the requesting board or agency regarding cards and mailing instructions.
